 Registration Notes:

1.     Players Pictures: IMHA reserves the right to publish / print player pictures & photos.

 

2.     Fundraising Contingency Fees: may be post-dated to March 1, 2021 if you plan on participating  in fundraising or included with the registration fee if you prefer not to fundraise. Fundraising cheques will be destroyed for all participants who earn their required credits.

 

3.     Late Fee: will be waived for new registrants ONLY.

 

4.     Acceptance of this registration form does not guarantee placement on a team. Players leaving Ilderton Minor Hockey Association after September 1st will be subject to a minimum retained registration fee of 50% unless a written request for consideration is received and approved by the IMHA Board of Directors. Requests to try-out for a team other then a player's regular age / gender classification is subject to Board approval. Requests must be made by writing a letter to the IMHA Board of Directors. Players trying out at a higher level must remain at that level.

 

5.     The Board of Directors will consider refunds on an individual basis upon receiving a written request by the participant or the participant's parents. Refunds will be granted to an injured player after 3 months regardless of whether or not the injury was hockey related. Requests for injury related refunds may be made at anytime throughout the hockey season. Refunds on a pre-season tryouts for any player who does not wish to continue will be given upon receiving a written request. There may be a fee per skate charged to the participant. All refunds are pro-rated over a 7 month period of time. There will be no refunds given after December 1, 2020.

 

6.     One parent from each household who has a player registered with the OMHA is required to complete the "Respect in Sport" parent edition. If you have taken the "Respect in Sport - Parent" in prior years then you do not need to take it again (it is a one time course). Anyone family who has not had a parent take it must do so by Aug 31, 2020 or your child will not be allowed on the ice until it is completed.

CODE OF CONDUCT – 2020 / 2021 HOCKEY SEASON
PLAYERS – PARENT – GUARDIAN


This Code of Conduct identifies the standard behaviour which is expected of all IMHA members including all players, guardians, parents, coaches, officials, volunteers, directors, officers, committee members, team managers, trainers, and administrators involved in IMHA activities and events.

The IMHA encourages an environment in which all individuals are treated with respect. Members and participants of the IMHA shall conduct themselves at all times in a manner consistent with the values of the IMHA which include fairness, integrity, and mutual respect for all.
During the course of all IMHA functions members shall avoid behaviour which brings the IMHA or the sport of hockey into disrepute, including, but not limited to, abusive use of alcohol, use of non-medical drugs, use of alcohol by minors, and use of profanity.

IMHA members and participants shall at all times adhere to the IMHA operational policies and procedures, to rules governing IMHA functions, and to the rules and regulations governing any competitions in which the member participates on behalf of the IMHA.

Members of the IMHA shall not engage in any activity or behaviour which interferes with a competition or with any player or team’s preparation for a competition, or which endangers the safety to others.

Members of the IMHA shall refrain from comments or behaviours (this includes the use of social media) that are disrespectful, offensive, abusive, racist, or sexist. In particular, behaviour that constitutes harassment or abuse will not be tolerated and will be dealt with under the IMHA Harassment Policy.

In the event of an alleged violation of the IMHA Code of Conduct, a written complaint must be submitted to the IMHA president.

Failure to comply with the IMHA Code of Conduct may result in disciplinary action. Such action may result in the member losing all privileges that come with membership in the IMHA including the opportunity to participate in IMHA activities and events, both present and future.

Ontario Women’s Hockey Association Rowan’s Law Acknowledgement Form

The Ontario Government has adopted the Statute “Rowan’s Law” that includes mandatory requirements for all sport, including the Ontario Women’s Hockey Association (OWHA). The OWHA’s Concussion Awareness Resource is available (link to be pasted) and includes Code of Conduct, Removal from Play, Return to Play, age-appropriate material and links to valuable additional resources.

The OWHA Concussion Awareness Resource must be read on an annual basis before an individual registers with the OWHA or with an OWHA member. The OWHA registration form requires a signature from every member/participant as well as parent/guardian for players under the age of 18.

OWHA member teams and associations are to provide the OWHA Concussion Awareness to every participant to read PRIOR to registering with the member team/association. This form may be collected by the OWHA member team/association as proof that the participant and parent/guardian if participant is under the age of 18 has read the OWHA Concussion Awareness Resource prior to registration.
