Health Form for Team Trainer I have attached the training form, if everyone could have it filled out and turned in next Saturday before game time. I will also print out some copies for everyone to have should they not be able to print off.

Socks will be available for sale at $20.00 per set. If you could bring in your cash as soon as possible, we will have a set for you. Also, it is important that the boys have new socks,(or at least in really good shape). This comes from Ilderton Minor Hockey. They will be looking. The socks are meant to be used only for games. Please indicate your need on the survey so that we can get them for the first game.
Name Bars for those that are new to IMHA, or require new name bars, they can be ordered directly from Pete’s Sports... 649 Oxford Street E, Phone: (519) 433-9555. You can just call in to them to order, just let them know you are with Ilderton Jets, and they will know your colours. Once you receive your Jersey next week, you can hand sew them on to the jerseys before our first game. (Blue namebar on blue jersey, white namebar on white jersey)
Mouthguards and
neckguards MUST BE WORN ON THE ICE AT ALL TIMES. They will not allow you on the ice without the protection. If you would like a custom made mouthguard, Caroline Filson can make them for around $30, you can contact her directly at 666-2009.
